Registration begins when your physician schedules your procedure. At the same time, a pre-admission testing appointment should also be scheduled. Generally, the physician will schedule this appointment, but patients must call 513.559.2199 to confirm an appointment has been made. During pre-admission testing, blood work and a general physical are done.

Your surgeon may ask you to have your family physician provide a History & Physical prior to your surgery. You are welcome to click here for a printable form that will appear on your screen. You can take with you on your next visit.

Pre-registration will also call patients prior to the procedure date to begin the registration process.
